I had this problem about three weeks ago. Tried to start the game but it never would launch.
Tried restarting the computer and maybe something else. Don't remember.
So I go online and find the recommendation to unplug both the mic and headset or speakers and plug them back in.
Fixed it straight away in my case. This surely would not work in all cases but it did this time.

I had an issue a lot the same as yours.
I had to goto control panel and switch audio from Digital to HD or something like that.
Recently I had an after a re-install that wouldnt allow my server list to show up.
I ended up installing gametracker lite which enables you to search for the server and join via that application.
Bombany also had the same difficulties and uses gametrackerlite now also

I wouldnt be able to play without it, for some reason my pc will not bring up the servers. Ive installed the game several times, might be a wondows eight issue?
Nothing came up in the forums so I had no choice but to use the gametrackerlite app
